I'm assuming you're needing the adapter for a boat trailer that has a surge or hydraulic coupler.
The 'brake' wire doesn't carry power to the trailer brakes, it merely 'locks out' the hydraulic coupler when the vehicle transmission is shifted into reverse so the trailer can be backed up without the trailer brakes coming on.

Typically the 5-Way Flat is used for trailers with surge brakes or hydraulic brakes. The additional wire is tapped into the backup lights to disengage the hydraulic trailer coupler (actuator) when the vehicle is reversing, thus turning off the trailer's brakes.
